Brackets Registration Form

The Brackets registration form consists of 4 parts.


  1. Team Information, including Custom Questions

  2. Contact Information for Primary & Alternate Admins

  3. Terms & Conditions (optional)

  4. Custom Content (optional)



Team Information:

The following questions are asked on the registration form. Tournament admins can add additional questions (See Below)


Team Name is a required field* and cannot be removed

Team website is optional and included by default but can be omitted

League website is optional and included by default but can be omitted

Checkbox-We are a local team and will not require accommodations is optional, included by default.

Special Notes an optional field and cannot be removed



To update the Team Website or League Website questions, click on the Configure Questions button at the top of the page and a window will open.



To configure the default questions or add Custom Questions, click on the Configure Questions button at the top of the page. Click on the line with your tournament name and a window will open.



At the top of the window are setting options for Team Website, League Website and Local Team. These settings can be omitted (Don't Ask), Optional (Team may or may not answer) or Required (must answer)




To add Custom Questions to your registration form, click on the "Add New Custom Question" button and a new window will open. From the Input Type drop-down box, select either "Any Text", "Numeric" or "Select List".

"Any Text" custom question requires a text based answer. Enable the Required check-box if you want this question mandatory. The Sort number will determine the position of the question on your from. 1 will be listed at the top. "Numeric" requires a number as the answer. "Select List" allows you to create a dropdown list. Only a single answer can be selected. In the "Options" line enter the your response items separated by commas. ie Yes,No,Undecided

Important:

If the question is set to Required, you will want to ensure every team registering will be able to provide an answer. Sometimes the answer you are looking for may not be available until later in the year. Making this question required could stop the team from registering.

# Alt Admins Req'd: The number of alternate admins required is typically set to 0. For example, if a coach is registering a team and hasn't selected a manager yet, they may not be able to provide an alternate admin. If this setting is 1 or higher, it would require the coach to add additional contacts they may not have at that point.


After a team registers, the tournament admin can view the answers to their custom questions in Brackets Control Panel. Go to Brackets—>Manage Teams—>Custom Questions. The answers can be exported to a spreadsheet on this page.


Example:

The following example is a registration form with the team website and league website omitted, custom questions Jersey Colour? How many players? and Can you play Thursday? (as a dropdown) added.

Contact Information: Primary & Alternate Admins:

The individual who registers the team is designated as the Primary Admin, typically the head coach or team manager. Up to two additional administrators, known as Alternate Admins, can be added during registration or later. Both the Primary and Alternate Admins receive all automated website communications and have equal access to the team account. This includes the ability to manage payments, upload documents, and book hotel room blocks through SportsHeadz Concierge. The phone number of the primary admin is a required field.

Terms & Conditions:

If your tournament has a legal disclaimer or refund policy, paste this information into the “Terms and Conditions” section. Before submitting the registration, the Primary Admin must confirm they have read and agreed to your Terms and Conditions. If the team is accepted, a copy of these Terms and Conditions will be included in the acceptance email sent to all Admins.


Custom Content:

The custom content box at the bottom of the registration form allows tournament organizers to share important information about the tournament that does not require acknowledgment or agreement from team admins.
